NPR: Medical Panel Recommends No-Cost Birth Control

Will all health insurance plans soon have to offer all FDA-approved forms of prescription contraception at no upfront cost to women? They will if Health and Human Services Secretary Kathleen Sebelius accepts the recommendations released today from an expert panel of the Institute of Medicine.
The study — released a day early — actually calls for eight additional services for women to be added to the list of preventive care patients should be offered with no cost-sharing. The new services include annual "well-woman" visits; screening of pregnant women for gestational diabetes; screening for sexually transmitted diseases, including HIV; more support for breast-feeding mothers; and counseling and screening for possible domestic violence.

World SafetyNet - Make It Bigger Not Smaller - NPR

More than 1 billion people in the world are living with some sort of disability, according to a new international survey. That's about 15 percent of the world's population, or nearly one of every 7 people.

The above is from the story by NPR which makes you wonder about Wisconsin and what Governor Scott Walker wants to do to Medicaid recipients in our state.  WWJD.

The report includes a foreword by theoretical physicist Stephen Hawking, who can't feed himself or get dressed or speak without assistance because of his amyotrophic lateral sclerosis, a debilitating and usually fatal disease. He says there's a moral duty to help disabled people.

NYTimes: Disabled, Abused, Dead - What's Ahead In Wisconsin??

Of course this story about what has been going on in New York will never happen in Wisconsin - we hope!  It does sadly highlight a sometime problem with "state institutionalization" and "for profit institutionalization".  The drive to save money, reduce costs, or whatever you call it always ignores the victim or equivalences the care provided by family with that provided for the "state or private entity".  We need both and should prefer family care and support those willing to provide it!

The the NYTimes:  A Disabled Boy’s Death, and a System in Disarray

... Yet on a February afternoon in 2007, Jonathan, a skinny, autistic 13-year-old, was asphyxiated, slowly crushed to death in the back seat of a van by a state employee who had worked nearly 200 hours without a day off over 15 days. The employee, a ninth-grade dropout with a criminal conviction for selling marijuana, had been on duty during at least one previous episode of alleged abuse involving Jonathan.

Health Care, SeniorCare, BadgerCare, Medicare - Budget Impacts

This excerpt is from a WI 1848 Forward blog entry I put together back in March 2011.  To see Steve's website just click on the title above.  At the time it was motivated by a Guest Opinion Column by Steve Salmon in the Wisconsin State Journal -


What does Governor Walker really intend to do? He wants more flexibility (block grants) from the Federal government, which I guess means he wants to decide how to spend the money, which I guess means he will decide who is eligible and what they are eligible for. I guess that is why he is also asking the Wisconsin Legislature to turn over administrative rule writing to him!

All seems consistent with what he said a year ago ...

Not a lot of content!  Makes a lot of statements that sound like facts but ...!

For example, from the source ...

We need to modernize not only Medicaid’s benefits and service delivery, but also its financing. In good times, the open-ended federal Medicaid match encourages states to overspend. Amazingly, the program is now viewed by some states as a form of economic development because each state can at least double its money for each dollar spent. That matching feature penalizes efficiency and thrift, since a reduction of $1 in state spending also means forfeiting at least one federal dollar, often more.

What does this mean?  I guess it means states waste a $1 so they can get another $1 ... I don't think so!

It is always so easy to claim something is old or outdated or inefficient!  When Governor Walker talks about those kind of things you have to be cautious about what he really means and rather than soundbites have a public discussion before he uses the legislature as his rubber stamp!

Do Not Forget ... he wants to write all the rules himself ... don't bother the legislature with any oversight!

What is ScareCrow Walker up to - Health Care Risks!

(4/21 thoughts) This blog entry below was written, mostly on 2/11, and updated for actual "blogging" on 3/2 - Walker: Budget Repair, Healthcare - Hidden Agenda, Retirees Beware .  It ranks right up there with some of the "scariest" things that Walker might try to do and that must urgently, even proactively be counteracted.  Only if the editors and journalists and public remain vigilant will another "budget repair" scare be prevented.  The major concern, among so many, is from a letter, quoted in that blog entry, that Governor Walker sent to all state employees.

From the blog entry -
Even more troubling, in a letter by the Governor to state employees, he directs -

“the Group Insurance Board to implement changes to health insurance plan designs to further reduce premiums by 5 percent and will implement health risk assessments for all state employees beginning on January 1, 2012. “

What does this mean? Sounds like a clever way of saying you are going make some employees pay more for their health care than others!? Older employees perhaps? Diabetic employees maybe? Employees with preexisting conditions? With Governor Walker putting himself in charge of agency rule writing and review – who knows? There are over 191,000 state annuitants, who may or may not have associated health care, which they pay for in full – are they going to be impacted?

The original "blog" context raised the alarm for annuitants - that they could find their health care coverage changed significantly even though they pay for it in full as an out of pocket expense.  If coverage options change for state employees and other governmental employees then almost certainly annuitants will be affected as well!

Let's repeat the quote from the letter and remove a little verbiage so as to focus more clearly on the scary part!
“the Group Insurance Board  ... will implement health risk assessments for all state employees beginning on January 1, 2012. “
What does this mean?  If "implementation" meetings are or will take place who will attend and when will they be held?  What information is on the record of these meetings.  What exhibits have been or are being produced?

How does one file a FOIA request before the fact or even after the fact about such "implementation planning?"

Back to the abridged letter from the Governor and what it means!  The original blog entry suggested, repeated again -

Get an education look at these links ... a two person family (my guess) in Wisconsin spends $6500 each in a healthy year (with pretty good coverage plus expenses) ... sort of confirms the National Geographic graphic ... so beware of your cost and if Walker or Ryan get the changes they are looking for ... ask if your life is more or less complicated or your health care costs you more or less

More thoughts - do any of the countries depicted have a Paul Ryan Health Care System?  Except for Mexico and the United States they all seem to have Universal Health Care.  We spend roughly $2900/yr more to die two (2) years younger than the Swiss.  As I recall the Swiss use private insurance but manage (regulate it) very uniquely?

Fixing Medicare: Obama Vs. Ryan (Who is he?) 4/18/11

So let me add a caveat about projections like this ... they are practically worthless!  An interesting exercise but the numbers in 2032 will not be even be close under either of Ryan or Obama plans!

Ryan's plan appeals to the "market place" to do the allocation ... if you can afford it you can have it!  The market place approach will have huge transactional costs to be borne by the consumer - these costs can be money or personal time trying to make a choice.  This is what Walker wants in Wisconsin.


This piece is worth evaluating carefully -
Fixing Medicare: Obama Vs. Ryan - Planet Money 4/13/2011

It starts with a graphic.  The dashed line on the graphic shows the expected revenues (government) out to what looks like 2032.  Stacked on top of one another from top to bottom is first Social Security, then combined Health (Medicare, Medicaid, CHIP and Exchange Studies), then comes Other Noninterest Spending (which I assume includes Military).  Then on a solid line above the dashed  Revenue line is the label Total Primary Spending.

The Revenue line being below the Total Primary Spending line implies deficits.  The first thing to notice is the combined colored area (Health) keeps getting wider as time goes by and the other areas between their respective boundaries remain the same width.  Can you put your finger on the problem?

In the long run, growth in the federal deficit is all about health care — particularly Medicare, which accounts for the biggest chunk of government health spending.
President Obama and Rep. Paul Ryan, the Republican chair of the House budget committee, both recognize this.  Both want to slow the rise in Medicare costs. But they want to do it in very different ways.

The whole thrust of this article is to illustrate the difference of approaches of Paul Ryan and the Obama Administration.  The way I interpret Ryan's plan is he makes the lines stop diverging, i.e., no growth in expenditures will be allowed.  Obama has to find a way to do this too!
Under the Ryan plan, according to the CBO's estimates, the government would pay less for Medicare, and individual seniors would pay more.
Under Obama's plan, the government, rather than individual seniors, would still be on the hook if health costs keep rising so quickly.
So let me add a caveat about projections like this ... they are practically worthless!  An interesting exercise but the numbers in 2032 will not be even be close under either of Ryan or Obama plans!

The Obama approach -
But the Obama plan aims to make it easier for the federal government to slow the rise in Medicare costs.
If costs kept skyrocketing, a special panel would come up with ways to "reduce the rate of growth ... while not harming beneficiaries' access to needed services." Congress would then have to accept the panel's recommendation or make other changes that achieve comparable savings.
What seems to be lacking in any of these discussions is what or how might the tax system be modified to moderate the assumed growth of health costs, or other more direct intervention to make the market place perform in a way we might like.  We use the marketplace to regulate things like airline routes, telephone service, radio and television - at least in ways different from today.  After all we have been deregulating for the last 40 years and where are we now.   Regulation no doubt has some rigor mortis problems but my right to live or die ... how should that be decided ... my genetics and my bank account, or the lottery.  The lottery  ... the more you spend on tickets the more likely you get health care - but you still have to win!
